Education
Education is the foundation of patients being able to self-manage their chronic kidney condition. MyRenalCare helps patients understand their disease, what to monitor and what to look out for by providing bite-sized videos. These are available on the MyRenalCare and our YouTube channel.
2. Engagement
MyRenalCare is not just an app, it’s a new way of working
MyRenalCare helps you be more engaged in your condition by supporting you to self-manage it. This includes enabling you to record your clinical observations and symptoms, and if you’re on dialysis (home haemodialysis and peritoneal dialysis, both continuous ambulatory peritoneal dialysis (CAPD) and ambulatory peritoneal dialysis (APD)), record your treatments. Your clinical team are able to immediately access this data and perform remote monitoring. They will conduct 100% digital consultations and provide you direct feedback via the platform.
This new way of working requires less face to face appointments, meaning less time out of work /reduced need for child care, less travel and reduced risk of Covid. Remember to inform your clinical team of any concerns that you may have - they can access your record - but it is important that you inform them of your concerns and seek professional medical help.
